New Orleans, LA – New Orleans East Hospital is pleased to announce a new partnership with Social Health Bridge that will help bridge the gap between their quality healthcare services and the social determinants of health to address the overall health and wellbeing of the Walnut Square community.

“As a result of this partnership, patients and their family members will have increased access to healthcare services and a wide variety of community-focused services supported by the onsite community health worker. Patients can rest assured knowing they have access to the high-quality patient-centered care New Orleans East Hospital is known for providing.” - Dr. Takeisha Davis, MD, CEO of New Orleans East Hospital.

“We are pleased to partner with New Orleans East Hospital to work towards our shared goal of reaching underserved communities and together we can strengthen our ability to serve local residents where they live,” said Laura Sankey, CEO of Social Health Bridge.

New Orleans East Hospital will offer onsite health screenings, telehealth assistance, home wellness checks, chronic disease management, education and resources to residents and community members, right at their doorsteps.
