Committed to providing the most convenient methods to fulfill your health care needs, Lakeside Hospital offers Online Pre-Registration, Mammogram Scheduling and Pre-Procedural History forms.

Pre-register now:

Simply complete the 10-20 minute online forms at least three (3) business days prior to your scheduled appointment. For privacy reasons, if you leave your computer idle for more than 20 minutes, your form will not be saved and you will have to begin again.

Pre-Procedural History

What you'll need to Pre-Register:

Your personal contact information including occupation information
Emergency contact information
Insurance information, group name and number, policy number, address and telephone number
Physician name
Date of procedure

Surgery patients, please also have the below information available:

The names, addresses and phone numbers of your physicians.
A list of all medications you are taking, their dosage and frequency.
A list of surgical procedures you have ever had
The name of the surgeon who will be performing your upcoming procedure

Check-in with our registration staff on the date of your scheduled service to complete the registration process. Please be advised that you will sign all consent forms during your visit.
